CONSENT & RELEASE AGREEMENT
1. I give my irrevocable consent to have my praise report (testimony) shared, including all files uploaded, without restrictions or limitations, across all platforms associated with Leon du Preez International (LDP), Prophet Leon du Preez, Encounter Church, and other ministers and partners of Prophet Leon du Preez’s ministry.
2. I also give my irrevocable consent to LDP, Prophet Leon du Preez, Encounter Church, and their broadcasters, publishers, licensees, and assignees (collectively referred to as the "Authorized Entities") to use and reproduce my testimony, without restrictions or limitations, in any teaching resources. This includes all editions and formats, transmitted or published in any medium, platform, or language worldwide, including any advertising and publicity related to these resources. I acknowledge and agree that there will be no limits to the usage of my testimony.
3. I agree to provide complete contact information so that LDP or its publisher(s) and/or licensees may contact me if needed.
4. I agree to allow my testimony, including all files uploaded, to be edited for language and readability, with the understanding that the editing team will make every effort to maintain its accuracy.
5. I agree to make no claim of any kind whatsoever (including but not limited to claims of libel, slander, invasion of privacy, right of publicity, and/or copyright infringement) against the Authorized Entities in connection with the use of my testimony.
6. I agree to having my name (as provided) revealed.
7. I confirm that I am 18 years or older, or that I have permission from my parent/guardian to share my testimony.
8. I confirm that my testimony is given voluntarily and that the information provided is true and accurate to the best of my knowledge.
